Southeastern Community College encourages students and all other members of the community to report concerns to the College.

If you are unsure which report to use, please contact the Dean of Students for guidance regarding your situation or concern at or (319) 208-5101.

Emergency Care

In the event of a life-threatening emergency, contact the police immediately by calling 911. Once the immediate health or safety concern has been addressed, please follow up by filing an incident report.

General Incident Report

This form is to be used by students, staff, and faculty to report alleged violations of the Student Code of Conduct  or any other behavioral concerns. Use this to report a specific incident of misconduct, including sexual misconduct, or any general concerns for an SCC student’s behavior.

Academic Misconduct Report

This form is to be used by students, staff, or faculty, to report alleged academic misconduct by SCC students. This includes, but is not limited to, acts of plagiarism, cheating, or any violation of an established academic policy or program regulation at SCC. Academic misconduct reports are addressed in partnership with Academic Affairs and Student Affairs.

CARE Team Referral

This form is to be used by students, staff, faculty, or community members to report any concerns for student well-being. The CARE Team meets regularly to discuss intervention plans for student support, referrals, and safety. The CARE Team also coordinates response to student behavior which requires intervention to help the campus community and its members feel supported and safe. This form can be used to report a specific incident or general concerns for a student’s welfare.

Student Complaint & Grievance Report

This form is to be used by students if they have a complaint regarding SCC, an SCC staff or faculty member, or an experience they had with any office at the College. Students are encouraged to first work informally to resolve the complaint, as is described further in the Student Complaint Policy. If the student is unable to resolve the complaint informally, they may use this form to file a formal complaint with the appropriate Dean or administrative supervisor.

Grade Appeal

This form is to be used by SCC students to formally appeal the final grade they received in a course.  Grade appeals must be initiated by the student no later than fifteen (15) working days after the grade “due date” (the final date when grades are to be posted by faculty members). Students are expected to read the Grade Appeal Policy AND attempt to contact the course instructor prior to submitting this form.

After Submitting a Report

Once a report is submitted, it will be reviewed and routed to the appropriate SCC employee or supervisor for follow up. Reports are typically reviewed within 24-48 hours of submission. Often, the next step in the process is to contact the person who filed the report to confirm it was received and to discuss next steps. Please remain available and amenable to providing additional information.

Please note that information shared in the reports becomes a part of students' educational records and is protected by a federal privacy law called the Family Educational Rights and Privacy Act (FERPA). As such, students may have access to their educational records, which includes access to the name of the person filing the report. Retaliation of any kind is strictly prohibited in the Student Code of Conduct.
