Security Incident Form


If you believe a security incident has occurred please complete the steps described below:

  1. Complete the "Security Incident Form" below and click submit. This form will be submitted to our ITS Network Manager, ITS Help Desk, and ITS Security Officer.
  2. Contact the ITS Help Desk at 319-208-5087 during normal business hours (7:30 am to 5:00 pm).
    Contact the ITS Security Officer at 319-750-9485 during non-business hours.
